Frequently Asked Questions (FAQs)
Hospice billing is the process of submitting claims for end-of-life care, mainly to Medicare under the Hospice Benefit. It covers daily per-diem rates by level of care, Notices of Election, benefit period tracking, and cap management, all filed on the UB-04/837I institutional claim format.
Hospice is paid as a daily per-diem rate, not fee-for-service. Agencies bill monthly for each day a patient is on service, at one of four levels of care. They must also file election notices, track benefit periods, and stay under the annual Medicare hospice cap.
The four Medicare levels are Routine Home Care (RHC), Continuous Home Care (CHC), Inpatient Respite Care (IRC), and General Inpatient Care (GIP). Each has its own daily rate. RHC also pays a higher rate for days 1–60 than for day 61 onward.
An NOE tells Medicare a patient has elected hospice. It must be filed within 5 calendar days of the election date. If it’s late, Medicare won’t pay for the days between the election date and the date the NOE was accepted. The agency absorbs those days as a loss.
Medicare sets an annual aggregate cap on what each hospice can be paid per beneficiary. CMS updates it every fiscal year. Agencies that go over must repay the difference. Monthly cap projections and length-of-stay monitoring help prevent a surprise repayment demand.
The usual causes are late or rejected NOEs, a missing or late face-to-face encounter, unsigned or missing certifications of terminal illness, incorrect level-of-care coding, and overlapping claims with other Medicare providers. AffinityCore’s hospice billing team catches these at intake and documentation review, before the claim is submitted, so they never turn into denials.
A face-to-face encounter is required before recertification for the third benefit period and every period after that. A hospice physician or nurse practitioner must complete it within 30 days before the new period begins. If it’s missed, the patient can’t be recertified, and those days are unbillable.
Hospice claims are usually submitted monthly, after the month of service ends. The NOE is filed at admission, and a Notice of Termination/Revocation (NOTR) is filed at discharge, revocation, or transfer when no final claim follows soon after.
For many small and mid-size hospices, outsourcing is worth it. A specialist partner like AffinityCore already knows the NOE deadlines, cap rules, and level-of-care documentation requirements. That reduces denials and avoids the cost of hiring and training an in-house billing team. Larger agencies sometimes keep billing in-house and outsource only audits or denial management.
Most charge a percentage of collections, while some charge a flat monthly or per-claim fee. Pricing depends on census, payer mix, and scope, such as whether cap management, eligibility checks, and denial appeals are included. AffinityCore offers a free RCM audit so hospices can see their current revenue leakage before choosing a pricing model.
